Workplace feedback planner
This feedback planner helps you prepare a specific work observation, its impact, and a request for the next step. It includes space for the other person’s explanation and an actual agreement. Use it for ordinary feedback preparation, not employee scoring or a substitute for workplace procedures.

- Follow-up date and what we will review:
Who it is for
Coworkers and managers who want to discuss a concrete work behavior without guessing motives or turning feedback into a personality judgment.
Prepare facts that can be checked
Write what happened and when. “The handoff document did not include the delivery date” is more useful than “You do not care about details.” Explain the work impact and check whether expectations or resources were unclear.
For example: “Without the delivery date, the next shift could not prioritize the request. Can we add that field to the handoff?” Then ask what made the field difficult to complete. You may learn that the date was not available.
Leave the agreement blank until you have talked
Your proposed request is not yet a shared commitment. Use the listening area to record context and then write the next step you both understand. If you have decision authority, explain which requirements are fixed and where there is room to choose.
Print the two-page planner for preparation and follow-up. Avoid personal labels and unnecessary sensitive details. Formal performance matters still need the relevant workplace process; this worksheet simply organizes an ordinary conversation.
